Family Enrollment
All forms can be completed within Adobe Reader. Simply type in the fields and print out the completed form. Some forms will not allow you to complete certain fields, as those must be filled out with a pen. Download Adobe Acrobat Reader Here.

STEP 4 - ADDITIONAL ITEMS TO BE BROUGHT TO ENROLLMENT APPOINTMENT
- Child's Yellow Immunization Card/Record
- Child's Blue Immunization Card (will be provided at enrollment appointment)
- Proof of TB Clearance (for anyone who will be volunteering regularly in the classroom)
- Emergency Kit (with all requested items in a Ziploc bag)
- Copy of Red ID Card (one will be issued to community families - must be submitted annually)
- Copy of Official SDSU Printed Class Schedule Showing Your Name (SDSU students only)
